Use this form if you are applying for Certification. Nobody may apply for certification unless they have completed the requirements.

Applying for any level of Certification is not a guarantee that someone will receive that level. Other levels, Recognition, or no certification, may also be granted based on proven performance.

Recommendation to Apply for  Certification:
* Must have actively officiated (not as an alternate) games of Roller Derby under the WFTDA Ruleset. If you do not meet these metrics, your personal history could still be enough! Please email certification@wftda.com to discuss. Everybody's story is different.
** Level 1: 10 roller derby games in the prior year
** Level 2: 20 regulation or sanctioned games, plus 10 additional games, in the past two years
** Level 3: 15 sanctioned games, plus 15 additional games, in the past two years

* Must have have the required Overviews of Officiating Skill (OOSes) on record, PRIOR to submitting this form.

*** One must be from a HEAD OFFICIAL
*** One must be from an ATHLETE or ATHLETE REPRESENTATIVE
*** One must be from another official: PEER, HEAD, or EXPERIENCED
*** Level 2 applications must include one additional OOS (beyond Level 1) from any other official
*** Level 3 applications, that "one additional OOS" (beyond level 1) must be from an EXPERIENCED official

** Level 1: Understood by your local area to be consistent in at least one role
** Level 2: Understood in your region to be excellent in at least one role and to have a general understanding of all roles
** Level 3: Understood to be world-class in at least one role and to have a holistic and textured understanding of the sport and how to officiate it across the board
** All Certified officials should ensure that they have addressed the points of feedback in their prior Certification Review Summary prior to applying for a higher level

Ideally this person is someone who has frequently been an athlete you were officiating. If no such person is available, someone who holds the athlete's perspective can also fill out this form. Do not submit this form until you have confirmed that they have submitted their report.
Head Official or Mentor, who has already submitted an Overview of Officiating Skill (OOS) on my behalf (Name & Email) *
Ideally this person is a mentor who has a broad view of your officiating history to this date, such as an official who has worked in a "head" role while you were officiating, either as your crew head, as your tournament head, or as the head official for your league. If no such person exists, choose an official whose guidance and feedback you respect and appreciate. Do not submit this form until you have confirmed that they have submitted their report.
Additional Official who has already submitted an Overview of Officiating Skill (OOS) on my behalf (Name & Email) *
Ideally this would be someone you have shared a crew with many times, or someone who has served on a crew for which you were the Head Official. Do not submit this form until you have confirmed that they have submitted their report.
Second Additional Official (Required for Level 2 and 3 Applications)
For Level 3 applicants, this must be an "experienced" official, who is Level 3, or has worked Playoffs for the WFTDA, MRDA, or JRDA.
